| Benefit Eligibility |
All full-time employees are eligible for benefits beginning the 1st day of the month, following your hire date. You have thirty days in which to enroll, but please keep in mind that your benefits will begin on the 1st day of the month as will your benefit premiums. Employees caring for their parents and claiming them as dependents on their income taxes cannot list them as dependents for company-provided benefits because tax and benefit dependency rules differ. For benefits eligibility, dependents are defined as being a spouse or child under a certain age. Our plans do not extend coverage to parents, even if they qualify as tax dependents. |
| 401(k) Eligibility | You are eligible to participate in the 401(k) plan on the first day of the month following your date of hire and if you are 18 years of age or older. |

Excelligence offers three medical plan options through UnitedHealthcare (UHC). Each plan balances premiums and out-of-pocket costs, with extra savings when using in-network providers. Employees can also manage benefits easily through myuhc.com and the UHC app.
HSA – High Deductible Plan
Lowest premium, best for those with minimal healthcare needs. Includes an HSA to save pre-tax dollars for medical expenses. Preventive care is fully covered; other services apply after the deductible.
HRA – PPO Plan
Balanced option with moderate premiums and deductibles. Excelligence helps offset costs with an HRA contribution. Copays apply for office and urgent care visits, while preventive care is covered in full. An FSA is also available.
PPO – Lowest Deductible Plan
Highest premium but lowest deductible, ideal for those with frequent medical needs. Offers predictable copays for office visits and ER care. Preventive care is fully covered, and an FSA is available.
Excelligence offers three dental plans through UHC, all focused on preventive care and cost savings with in-network providers.
PPO Dental – High Plan: Full preventive coverage, strong benefits for basic and major services, plus orthodontia.
PPO Dental – Low Plan: Lower premium option with full preventive coverage, reduced benefits for other services, no orthodontia.
DHMO Dental Plan: Lowest premium, in-network only, set copays for services, and reduced-cost orthodontia. No deductible or annual maximum.
Our vision plan, offered through VSP, supports eye health and early detection of medical conditions. It includes low-cost annual exams, full coverage for standard lenses, and an allowance for frames or contacts. Employees save more when using in-network providers, who offer the richest benefits and lowest out-of-pocket costs.
Excelligence provides company-paid Basic Life and AD&D Insurance through New York Life at no cost to employees. Coverage includes a flat benefit amount, with no evidence of insurability required. Employees are responsible for keeping beneficiary information updated in iSolved.
